IMPORTANT NOTES FOR PARENTS:
- This form is only to be completed by legal parent/guardians wishing to apply at a school other than their home or catchment school. Legal parent/guardians intending to send their child(ren) to their designated catchment school do not need to complete this online form. If a child has a legal parent/guardian who lives in the desired school catchment area, please use that address to register the student.
- What's my home, or catchment school?
Your home, or catchment school, is determined based on your primary residential address. CLICK HERE if you need to verify your home school using our School Locator tool.
- You are permitted to submit your child's application for only ONE out-of-catchment school's waitlist.
- If you submit multiple applications for different schools for the same child, only your first application request will be considered. All other applications will be discarded. Please do not submit multiple requests.
- Upon submitting the form, you will receive an email confirmation of your application request. If there are errors in this request, please email registrations@sjasd.ca to have corrections made to your submission.
- Providing false, misleading or incomplete information on this form will constitute just cause for termination of the request. By submitting this form, you understand and agree to abide by the School Division policies and Provincial legislation with regard to Schools of Choice.
- Legal parents/guardians will be notified of the status of their request by Friday, June 14, 2024. Further, parents must register at their home catchment school prior to the final notification of the status of this school of choice application.
- The personal information provided as a function of this application request is collected under the authority of the Public Schools Act and of the Freedom of Information and Protection of Privacy Act (FIPPA) and will be used for the purpose of managing out-of-catchment registration requests for September 2024. By completing this request, I understand that any other use or disclosure of this personal information will not be permitted without my consent unless authorized under FIPPA.
