NOTE: The apostolic sender is responsible for the contents of this form as leader

of the sending church, the fiscal sponsor and fiduciary organization.

HIM CHURCH PLANTING ROLES:

APOSTOLIC SENDER: The HIM Apostle who has commissioned the planting of a new church.
APOSTOLIC CHURCH PLANTING CENTER (ACPC): The church/ministry approved to plant HIM churches.
PLANTING LEADER: The Leader of the church that is being planted.
HIM CHURCH PLANT (HCP): The church that the Apostolic Sender has commissioned.
