We will ask for further information from you if this is required to complete setting up your support.

If your request for support is needed within 7 days, please complete this and also contact Disability Services. You can do this by emailing supportworkerteam@leeds.ac.uk or phone 0113 343 3927.

Once this is submitted, a member of Disability Services will be in contact to inform you whether your application of funding has been accepted.

If funding is approved, a member of Disability Services will be in contact to set up your support.
