This online form is for Marketplace Vendors only.  If you are a food vendor, you must fill out the Taste of Asia Vendor online application.
BEFORE COMPLETING THIS FORM, CHOOSE 2-3 ELECTRONIC PHOTOS OF THE ITEMS YOU WILL SELL and HAVE THEM READY TO UPLOAD.  ALSO HAVE YOUR CREDIT CARD NUMBER AVAILABLE TO PAY FOR THE $35 NON-REFUNDABLE APPLICATION FEE (Non-Profit organizations application will be waived  at check-out)

BOOTH CATEGORIES & FEES

CHOOSE YOUR CATEGORY TYPE:
MARKETPLACE BOOTH
This category includes businesses/organizations selling Asian related goods/services and that reflect Asian culture and traditions. If your business/organization will be offering health services/information and free screenings to the public, you will be considered for the Wellness Village.
NON-PROFIT BOOTH
Your business/organization must be a 501(c)(3) non-profit.  A copy of your IRS Letter of Determination 501(c)(3) non-profit status must be provided with submission of vendor agreement, if accepted as vendor.  The $35 application fee is waived at check-out.
COMMUNITY BUSINESS BOOTH
If you do not fully fit the above Marketplace vendor guidelines, perhaps this area is for you. A limited number of Community Business booths will be available to applicants meeting all guidelines. Booths will be assigned on a first come, first served basis. This category may apply to those selling products and services through independently owned/operated community businesses or consultants that sell independently.

APPLICATION AGREEMENT

I have read and agree to the Marketplace Vendor Requirements posted on www.cdbf.org.  I also agree to pay the non-refundable $35 application fee with this online application and understand that my application will not be accepted without the application fee.  This fee is waived for non-profit organizations.
