Please read through ALL information carefully before signing:
1) The student and the parent/guardian signing this Permission Form acknowledge that the post-graduation trip at Dave & Busters is not being sponsored by and is not an official event of either Thunderbird High School or the Glendale Union High School District. Instead, the trip is being sponsored by the Thunderbird Parent Association, a not-for-profit Arizona corporation (hereafter referred to as "TPA").
2) After graduation, students must report to the Desert Ridge Dave and Busters located at 21001 N. Tatum Blvd. Suite 44-1400, Phoenix, AZ 85050 at 11 pm on May 20th, 2024.
3) Students are NOT permitted to leave Dave & Busters after they have entered the building until it is time to depart or they are approved to leave with a parent or guardian.
4) Students must abide by the behavior policies established by the TPA and must comply with the directions and directives of the adult chaperones that will be accompanying the students on this trip. Additionally, the students must comply with all rules and regulations and any applicable code of conduct of Dave & Busters, and students must comply with the directives of the Dave & Busters staff.
5) Thunderbird's dress code guidelines will be used and will be strictly enforced.
6) Students may be subjected to a search for alcohol, drugs and other contraband before entering Dave & Busters.
7) Any student apprehended for a violation of law will be left in custody of local authorities.
Parents will be notified and will be responsible for arranging student's release and transportation.
8) There are NO refunds.
