Award Application

Eligibility Criteria
- Nominees must be a resident of Hastings or Prince Edward County and be between the ages of 15-21
- Nominees should be actively volunteering their time and talents to benefit the community.
- Nominees should exemplify volunteerism and leadership within the community programs.

Nomination Procedure
Nominations may be submitted by:
1. By a community organization, please include support letters with the nomination.
or
2. By a sponsor, who will be our contact throughout the process, please include support letters with your nomination.
Please note: nominations are made annually. Past nominees may be re-nominated

Guidelines:
-Nominations can be made annually. Past nominees may be re-nominated
-The nominees will be judged based their overall volunteer contributions and leadership. Please provide as much detail as possible
-Multiple letters of support can be provided

Letters of Support should speak of the youth's to agencies and community. Nomination can include letter(s) of support to be considered by the selection committee. There is no limit on how many letters of support you can provide.

A description of the nominee’s volunteer activities (past and present) include all significant instances.

A full description of the contribution that has been made to the business and community by your nominee. How have they implemented volunteerism as a value.
