Changing companies does not directly result in the loss of approval status as a Verifier. However, we must change your information in the Gateway and ensure you are connected to the correct VB. The purpose of this form is to notify the VOO of the change. You should complete this form and follow the emailed instructions to demonstrate that you remain eligible with the prescribed criteria and all technology systems are properly updated.
