Changing companies does not directly result in the loss of approval status as a Verifier or Trainer. However, changing companies impacts maintenance of certain criteria (e.g. insurance) and the appropriate contacts. The purpose of this form is to notify the VPM/TPM of the change. You should complete this form and follow the emailed instructions to demonstrate that you remain eligible with the prescribed criteria and all technology systems are properly updated.
WARNING: Verifiers must not create a new/separate Worldly account related to company change or employment cessation. Failure to comply may result in resetting of Verifier status and require both new application and retaking of training.
