The Team Mentor's Survey asks about aspects of the project experience and collects information about student team members. We appreciate you taking the time to tell us about your team's experience and your project—regardless of the result.

When you submit this form, each student team member will receive an email with instructions and link to submit the "Team Member's Survey." We encourage you to confirm with your mentees that they received the emails.

This form is for 2023-24 Academic Year projects.

This form will close at 11:59 p.m. on April 19; extended to April 26, 2024.

The Team Member's Final Survey will close on May 3, 2024.
