GP Referral - Secure document upload

Securely upload a referral or other documents for Prof Harvey or Dr Maud here.
 
Please note that Prof Harvey only accepts the following referrals:
 
 o Patients under the age of 40 years for the assessment/treatment of ADHD (priority is given to 18-26-year-olds in education)
 o Patients seeking a WPATH support letter for gender-affirming surgery
 
 
Dr Maud only accepts referrals for:
 
 o Obsessive-Compulsive Disorder
 o Women's mental health (menstrual, perinatal, menopausal)
 o Dr Maud prioritises referrals for women's reproductive mental health & OCD but will also consider accepting referrals for "other reasons" (including ADHD).

Please note that we no longer accept faxed documents.
 
This form is for GPs, practice staff and hospitals/other clinicians only.
 
The referring doctor MUST have seen the patient being referred at least once face to face.
 
Only documents in PDF or RTF format will be accepted. Please ensure the file is not encrypted or password protected.
 
A secure message outcome will be sent once your upload is reviewed by the psychiatrist.

Due to unprecedented demand, Prof Harvey has had to stop accepting referrals for new patients with ADHD. Please double check the reason for referral before submitting this form.
