Step 4: Record Your Volunteerism

If you plan to apply for the Community Award or the Community Service Award, you will be required to complete 56 hours of volunteerism. In order to document your volunteerism, you must provide signed Letters of Attestation, available for download here.
 
You may upload your signed Letters of Attestation after each event. You can save them here, and return to upload more signed Letters as you complete additional volunteer hours.
 
We recommend you scan or a take a photo of the signed Letter. All text and signatures must be legible in the photo.
 
Files we accept are .PDF, .JPG, and .PNG. Each file must not exceed 2 megabytes (MB) in size. If you have larger files you'd like to compress, we may recommend using a helpful online tool such as compressjpeg.com, compresspng.com, or smallpdf.com
