Owner Agreement

I hereby certify that my dog(s): is/are in good health and has/have not been ill with any communicable condition in the last 30 days. I have read and understand the following:

 
  1. I understand that I am solely responsible for any harm caused by my dog(s) while they are attending Tri-State Pet Care Boarding facility, Grooming and/or Doggy Day Camp or harm caused to themselves while staying in overnight accommodations. I understand I am solely responsible  for any damages to accommodations, and I will be charged for any repair necessary. 
  2. I further understand and agree that in admitting my dog(s) to the Camp, Tri-State Pet Care staff have relied on my representation that my dog(s) is/are in good health and has/have not harmed, shown aggressive, or threatening behavior towards any person or any other dog.
  3. I further understand and agree that Tri-State Pet Care, their staff, and volunteers, will not be liable for any problems that develop. Provided reasonable care and precautions are followed  and I hereby release them of any liability of any kind whatsoever arising from my dog’s/dogs’ attendance and participation at the Camp or stay in the overnight accommodations.
  4. I further understand and agree that dogs can sometimes receive minor cuts and scratches at daycare and any problems that develop with my dog(s) will be treated as deemed best by staff and volunteers of Tri-State Pet Care, at their sole discretion, and that I assume full financial responsibility for any and all expenses involved. 
  5. I understand that due to the social nature of our environment, there are some inherent risks. These risks include, but are not limited to: transfer of communicable diseases such as Canine Cough. The Kennel requires proof of current Rabies, Distemper, and Bordetella Vaccinations, the Bordetella Vaccine is not 100% effective. It is recommended that it be give to your dog at least 2 weeks prior to your pet's arrival date to increase efficiency. Kennel makes no guarantees in regards to Bordetella Vaccine.
  6. Should I choose to board without Bordetella Vaccine I assume the sole risk in doing so.
  7. If Grooming, I understand that Tri State Pet Care puts my pet's comfort above all else. In the event that my pet's coat is matted, I understand that the groomer may have to shave the matts out rather than perform a painful dematting procedure. I also understand that if my pet is severly matted, that there is an increased risk for clipper burn or cuts to occur. I understand that all attempts will be made to prevent this, however in many extreme matting conditions, it is unavoidable. I Understand that matted pets take additional time to groom so there will be an additioanl fee added onto the regular grooming price if my pet's coat is matted.  

Reservation policy details

Your reservation is not confirmed until receipt of a non refundable deposit of $100. If your invoice is less than $100, the full payment is due instead. This payment does apply towards your invoice total and is due within 48 hours of our acknowledgement via email or your request will be removed from our records.

Cancellation policy 

If you cancel at any time, you will be forfeiting your $100 deposit. Therefore, it will not be refunded or credited to your account.

You may choose to keep a credit card on file for deposits. Alternatively we will send an invoice via email for your required deposit payment that is due within 48 hours, or your request will be removed. 
By submitting your credit card details you are authorizing Tri State Pet Care to charge $100 deposit. If your invoice is less than this amount, we will charge the full payment instead.

In order to remain flexible with drop offs/pick ups between the hours of 9am12pm,(12-4 CLOSED) 5-7pm. You must schedule a drop off and pick up time before dropping off and picking up. You will receive a confirmation of receipt of this request and at that point you may make an appointment. Thank you and please contact us with any questions. Thanks!
