Adopting A Dog Can Be A Big Responsibility...
I understand that the adoption fee is $400.00 and covers all age appropriate shots, Rabies Vaccine, Microchip & Spay/Neuter. Spay / Neuter will only be covered by the Rescue if it is completed at an approved rescue vet location. No discount will be applied to the adoption fee, if I choose to vet my dog at an unapproved location. Vet appointments are scheduled in advance and take effort and time from one of our FNF volunteers. I understand that IF I am not able to make it to the scheduled appointment, I may be responsible for covering the costs of the spay/neuter myself.

All vetting appointments must be scheduled through Forgotten, Now Family Rescue. I understand that failure to show at scheduled appointment, without 72 hrs prior notice to Forgotten, Now Family Rescue, any future vetting will be at my expense & not covered by the rescue.

I understand that if the information I have provided in the adoption application is found to be false, my application may be returned and the dog shall be relinquished to Forgotten, Now Family Rescue without a refund of the adoption fee.

I understand that Forgotten, Now Family Rescue reserves the right to refuse any adoption of any animal to any person.

I understand that dogs adopted from Forgotten, Now Family Rescue come from a variety of sources and may have health or temperament problems that have not been observed while the dog was in foster care.

I agree to license my dog annually/keep the rabies immunization up to date, display the license and rabies tags on my pet's collar at all times, and to keep my dog properly confined or under reasonable control at all times.

I agree to return this dog to Forgotten, Now Family Rescue if I decide at any time, for any reason, that I can no longer care for the dog.
